• Who is the corporate sponsor of the project these consultants are working on?

• Do we have an NDA or confidentiality agreement with the consulting company?

• What is their name? Please make sure spelling is correct. -

• What is the consultants email? Please make sure spelling is correct. -

• What company do they work for? -

• What is their department? -

• What is their title? -

• Who do they report to (their Versalift/Time manager only)? -

• Are they going to be a remote user? -

• If you selected no to the above Remote User Question, where will they be located? -

• Will they need a PC or laptop? (If yes the user will be assigned local vs AVD Infrastructure setup) -

• Will they need an email account created? -

• Will they need Zone? If so, what do they need access to? -
